Clay County Health Department Seeks Liberty Citizen Input

The Liberty Community Health Action Team (LCHAT) is partnering with the Clay County Public Health Center and the City of Liberty to conduct a Health Impact Assessment regarding street design and having sidewalks on one or two sides of the street, and its impact on walkability and connectivity, and thus, the impact on the health of Liberty residents.

We appreciate your help to answer and disseminate this survey  through your contacts, ONLY among the residents of the City of Liberty.

Please fill out the survey by April 25, 2016. We will be using this information as part of a written report for the City of Liberty regarding city ordinances for future development.
